Coordinator, Gay & Bisexual Men’s Services – 1FTE at Queens Pride House

The incumbent is responsible for the coordination of comprehensive health promotion services directed at Latino gay/bisexual men, including access to comprehensive care, individual and group health education and risk reduction counseling, STI/HIV antibody counseling, testing and referrals, outreach and community building/leadership activities.

Principal tasks include, but are not limited to: Develops short and long term works plans for the execution of program activities and services (EHP & Open Colors, HIVCTR), inclusive of health promotion, personal growth, and leadership and community development. Participates in program development, evaluation and quality assurance efforts. Actively enforces policies and procedures governing services delivery, physical plant and staff performance. Develops and maintains meaningful collaborative relationships with other services providers aimed at improving comprehensive service delivery to Latino gay/bisexual men. Supervises, retains, evaluates the performance of the staff and coordinates training aimed at competency development. Coordinates program staff into a cohesive team well suited to meet HAF’s health promotion aims for gay/bisexual men. Facilitates and maintains the creation of safe spaces for Latino gay/bisexual men. Oversees daily implementation of project goals and services consistent with established standards and protocols and prepare required internal process and outcome program reports. Oversee the collection of client data and documentation and assessment of program activities.

Master’s degree in public health, social work, or related field plus 5 years filed experience in conducting health promotion services, community organizing and public policy advocacy related to the LGBT community. Bilingual (Spanish/English) and bicultural competencies and certification as an HIV Counselor are a must. (or certify within 90 days of hire. Candidate must possess a high degree of organization, excellent communication and interpersonal skills, and role-modeling ability; and is an exceptional team player.
Salary: $50K-$60K

Life & Wellness Coach – 1FTE each at Latino Pride Center – Manhattan, HAF – Bronx and Queens Pride House

The incumbent is responsible for providing individual psycho-educational support counseling and facilitating support groups to PLHIV/AIDS geared at alleviating stressors associated with an HIV diagnosis. Develops, coordinates and conducts educational workshops focused on promoting knowledge and skills among PLHIV/AIDS manage their health and overall living conditions in the Bronx.

Principal Tasks include, but are not limited to: Conduct participant screenings, initial intakes and psychosocial assessments / evaluations. Develop and implement a goal oriented intervention plan for each participant. Coordinate linkage and access to other clinical services, such as psychotherapy, psychiatry, etc. Facilitate/participate in internal and external case conferences with other agencies for optimum services delivery/benefit. Maintain detailed participant charts, i.e., document all client contacts and services provided in the client case record/forms and HAF’s data management systems. Conduct periodic reassessment, service plan update and implementation. Coordinate with case management providers for other ancillary services. Facilitate psycho-educational groups.

Master’s in Social Work or related field is required in addition to 3 years’ experience providing supportive counseling services to gay/bisexual men and transgender women living with or at high risk for HIV infection. Certification as an HIV Counselor a must. If not already certified, must complete certification process with 90 days of hire. Bilingual/bicultural competencies (Spanish-English). Teamwork, exceptional interpersonal and organizational skills.
Salary: $45K-$55K

Community Resources Specialist – 1FTE at Queens Pride House

The incumbent is responsible for providing short-term general assistance to PLHIV and those at risk in securing community social, medical and financial resources. Principal tasks include, but are not limited to: Recruits, engages and retains program participants LHIV at moderate to high risk for poor health outcomes. Conduct screening, assessments and service planning with case load commensurate with needs to the participants. Advocates for the provision of services and provides escort, interpretation services. Facilitate and track referrals for services commensurate with assessed needs. Conduct brief educational intervention on selected topics, i.e., secondary prevention, HIV disease management, entitlements and self-advocacy. Co-facilitates comprehensive health workshops and other educational seminars.

MSW desired and /or BA or BS in social work or relate field, plus 3-year minimum of practical experience in service delivery to PLWA/HIV and the provision of case management services; experience in assessing and advocating client needs, developing services plans, coordinating service delivery, and providing health counseling to persons with HIV/STI. Bilingual (Spanish/English) and bicultural competencies required.
Salary: $39,500 -$45K

Sexual Health & Wellness Counselor – 1FTE at HAF – Bronx

The incumbent is responsible for developing / retaining an active case load of 100 men who have sex with men who will receive comprehensive sexual health and wellness education, counseling and support services in order to effect improved health outcomes. Principal tasks include, but are not limited to: Recruits, engages and retains program participants at moderate to high risk for poor health outcomes, including HIV infection. Conduct screening, assessments and service planning with case load commensurate with needs to the participants. Administers Sexual Personal Assessment Surveys (SPAS). Screens, educates and navigates candidates for Pep/PrEP to appropriate providers. Facilitates curricula based HIV/STI multi session groups. Co-facilitates sexual health workshops and other educational seminars. Administers an assigned monthly quota of HIV / STI Testing Services to individuals at risk for HIV infection. Coordinates and tracks referrals to support and other services as indicated by assessed needs. Conducts motivational interviewing with the goal to help to affect change in behaviors that place individuals at risk.

Bachelor’s Degree in sexual health education or related field, plus 2-year proven track record of successful implementation of HIV/Sexual Health education to gay/bisexual Latino men. Bilingual (Spanish/English) and bicultural competencies required.
Salary: $37K -$45K

Health Education II – 1FTE each at Latino Pride Center-Manhattan and HAF-Bronx and Queens Pride House – Queens

The incumbent is responsible for the implementation and delivery of HERR and wellness education services, including HIV CTR services, principally to gay/bisexual Latino men at moderate to high risk for poor health outcomes, including HIV infection. Principal tasks include, but not limited to: engage, administers and assigned monthly quota of HIV/STI screenings to individuals at risk for infection. Conduct initial screenings, intakes and needs assessments with potential program participants. Coordinates and tracks referrals to health and support services as indicated by assessed needs. Promotes HIV/STI CTR Services among target populations. Conducts individual / group curriculum based sexual health and general wellness education sessions. Administers Sexual Personal Assessment Surveys (SPAS). Organizes and co-facilitates sexual health workshops and other educational seminars. Conducts targeted outreach in sex clubs, gay bars, gay cruising areas, “hot spots”, internet, etc.

Bachelor’s Degree in sexual health education or related field, plus 2 years proven track record of success on individual and group counseling in HIV required. BA may be substituted with an Associate’s Degree and 5 years of work experience in the field. Bilingual (Spanish/English) and bicultural competencies required.
Salary: $35K -$39,500

Youth Counselor – 1FTE each at Latino Pride Center-Manhattan and Queens Pride House – Queens

The incumbent is responsible the implementation of the Open Colors Program, an LGBTQ youth development program. Open Colors is a peer led initiative that draws from the experiences, interests and leadership of young Latinos to nurture and promote healthier sexual lifestyles among their peers. Tasks include, but are not limited to: Engage, screen, recruit, and retain participants in the program. Provide like skills building support / counseling to program participants. Conduct needs assessments and preparing/implementing a goal oriented services plan. Co- implements peer-training program and peer-led educational activities. Facilitate health and risk reduction counseling and education (individually or in groups). Facilitate and track referrals for services commensurate with assessed needs. Maintain up to date and accurate participant charts. Lead Staff-Peer Led community education forums, workshops and community engagement activities. Conducts pre and post HIV antibody test counseling. Promote healthy sexual practices in designated venues where Latino LGBTQ youth congregate by disseminating safer sex kits, HIV educational and other program/services marketing materials.

BA Degree plus 2 years’ experience in HIV/AIDS and youth services required. The candidate shall possess demonstrated ability to actively participate in the implementation of social-educational services designed for LGBTQ youth. Bilingual (Spanish/English) and bicultural competencies a plus.
Salary: $35K -$42K

Youth Leader – P/T at Latino Pride Center-Manhattan

The incumbent is responsible for assisting in the implementation of the Open Colors Program, an LGBTQ youth development program. Co-facilitates community education forums, workshops and other educational activities with youth, parents and community. Leads peer-led educational activities. Coordinates and promote healthy practices in designated venues where Latino LGBTQ youth congregate.

AA Degree plus 2 years of relevant experience in HIV/AIDS and youth required. AA degree may be substituted with high school diploma and 5 years’ experience. The candidate shall possess demonstrated ability to actively participate in the implementation of social-educational services designed for Latino LGBTQ youth. Bilingual (Spanish/English) and bicultural competencies a -plus.
